The International Foundation for Integrated Care (IFIC) and the Vrije Universiteit Brussels (VUB) is delighted to announce our 14th INTERNATIONAL CONFERENCE ON INTEGRATED CARE, “Taking Integrated Care Forward: People, Policy and Practice”. The conference will be attended by 350 international integrated care experts, including clinicians, policy makers and academics and will be realized from 2 – 4 April 2014, at the Diamant Conference Centre, Brussels, Belgium. International Foundation for Integrated Care (IFIC) The International Foundation for Integrated Care (IFIC) is a not for profit network that crosses organisational and professional boundaries to bring people together to advance the science, knowledge and adoption of integrated care policy and practice. The Foundation seeks to achieve this through the development and exchange of ideas among academics, researchers, managers, clinicians, policy makers and users and carers of services throughout the World. 14th International Conference on Integrated Care: “Taking Integrated Care Forward: People, Policy and Practice” This major international conference acts as a platform for senior professionals in the field of integrated care and long-term condition management to exchange knowledge, experience and new ideas. The 14th international conference will focus on 4 important areas of integrated care design and delivery: Designing policy to support integrated care: design and communicate national and local policies to support the drive towards integrated care. Supporting integrated care in practice: create systems of care that are people-centered; better understand the role of care coordination; achieve the best value; work towards evidence-based practice; monitor and evaluate success; implement quality improvement strategies. Managing the transition towards integrated care: leadership and management strategies; the usefulness of complexity, systems and network theories when managing change; the use of data and information systems to measure progress and outcomes; the developmental process of achieving integrated care. Thinking differently - innovation and new technologies: the use of information systems and new technologies to support people with complex needs; best practice in information management and data exchange; transforming health outcomes using new technologies.
